Phosphorus Still High? 3 Tips for Cats with CKD

Is your cat’s phosphorus level still high despite using a phosphate binder? Before increasing the dose, first check whether your cat is on a phosphorus-restricted renal diet and whether metabolic acidosis is present.
In many of the cats we have managed, addressing these two factors made a greater difference than simply increasing the binder dose.
Why High Phosphorus Matters
As kidney function declines, the kidneys become less able to remove phosphorus from the body. As a result, phosphorus builds up in the blood.
Persistently high phosphorus can accelerate CKD progression and lead to a faster loss of remaining kidney function. Kidney tissue that has been damaged and replaced by fibrosis generally cannot be restored.
The goal of CKD care is therefore to protect the remaining kidney function and slow further progression. This is why phosphorus control matters.
What 4,500 CKD Consultations Taught Us
Greycoat Research has provided more than 4,500 consultations for cats with Stage 2 and Stage 3 CKD. Through these consultations, we have helped manage and stabilize high phosphorus levels in hundreds of cats.
One pattern appeared repeatedly:
Simply increasing the phosphate binder was often not enough.
When metabolic acidosis was also present, identifying and managing it often produced a more noticeable and sustained improvement than simply increasing the binder dose.
Here are the three steps we consider most important.
1. Check the Renal Diet First
Phosphate binders reduce the amount of phosphorus absorbed from food. However, if a cat continues to eat regular food, high-phosphorus treats, or toppers, a binder alone may not be enough.
For cats with Stage 2 or Stage 3 CKD, phosphorus management should begin with a phosphorus-restricted renal diet. If phosphorus remains above the target range after your cat has been consistently eating and tolerating a renal diet, a phosphate binder may then be considered.
However, never allow your cat to go without food in an attempt to enforce a renal diet. Transition gradually while monitoring appetite, body weight, and daily food intake.
2. Check for Metabolic Acidosis
As CKD progresses, the kidneys may become less able to remove acid from the body. When acid accumulates and blood bicarbonate levels fall, this is called metabolic acidosis. The bones act as an important buffering reserve for the body.
When metabolic acidosis persists, the body may use carbonate and phosphate stored in the bones to help neutralize excess acid. During this process, minerals from the bones—including phosphorus and calcium—may be released into the blood. Cats with reduced kidney function may not be able to remove this additional phosphorus efficiently.
As a result, phosphorus may remain high even when dietary phosphorus is restricted and a phosphate binder is being used. A phosphate binder reduces phosphorus absorbed from food. It does not directly prevent phosphorus from being released from the bones during metabolic acidosis.
In our experience, identifying and managing metabolic acidosis has often been a major turning point in stabilizing high phosphorus levels. Ask your veterinarian whether your cat’s bicarbonate level has been checked. Your veterinarian may also consider checking chloride and potassium or performing a blood gas test when appropriate.
Metabolic acidosis should be confirmed through blood testing and managed under veterinary supervision.
3. Ask About a Sevelamer-Based Phosphate Binder
If phosphorus remains above the target range after optimizing the renal diet and managing metabolic acidosis, a phosphate binder may be needed. Rather than choosing one on your own, ask your veterinarian whether a prescription sevelamer-based phosphate binder may be appropriate for your cat.
Sevelamer binds dietary phosphorus inside the digestive tract and reduces the amount absorbed into the body. In the cases we have managed, sevelamer-based binders showed the most consistent and reliable results, including in many cats that had not responded sufficiently to other binders. Results can vary from cat to cat, so sevelamer should be prescribed and adjusted by a veterinarian.
Phosphate binders should generally be given with food, and blood phosphorus and calcium levels should be monitored regularly.
Remember These Three Steps
If your cat’s phosphorus level remains high despite using a phosphate binder, do not simply increase the dose.
1. Start with a phosphorus-restricted renal diet.
2. Check for metabolic acidosis.
3. Ask your veterinarian about a sevelamer-based phosphate binder when needed.

Start with a renal diet, manage metabolic acidosis, and use the right phosphate binder when necessary.
Phosphorus control is only one part of daily CKD care. Consistent nutrition, regular blood testing, and an individualized care plan also matter.
